Case Management Nurse (Healthcare)

Location: Fully remote, but candidates must be US based; a quiet workspace, reliable internet, and compliance with HIPAA and company security protocols are required.

What you will do

• Coordinate healthcare services and communication among providers to deliver comprehensive, patient-centered care.
• Assess medical needs and develop individualized care plans aligned with patient conditions and health goals.
• Advocate for patients, support access to appropriate services, and provide education on medications, treatment plans, and self-care.
• Monitor patient progress, adherence, interventions, outcomes, and resource utilization; adjust care plans when needed.
• Document assessments, care plans, and outcomes accurately and generate patient status and care outcome reports.
• Promote preventive care and wellness while ensuring compliance with URAC, CMSA, federal, state, local, and company standards.

Requirements

• Active, unrestricted Registered Nurse license in the state of residence; a Compact License is accepted for residents of compact states.
• At least 3 years of nursing experience in an acute care setting.
• Strong critical thinking, sound judgment, attention to detail, organization, and ability to work in a deadline-driven environment.
• Excellent communication and interpersonal skills with a collaborative approach to care coordination.
• Must be US based and able to comply with HIPAA and company security policies.

Nice to have

• Compact State Nursing License.
• Case management experience.
• Bilingual Spanish skills.
• CCM, CCP, or another relevant certification, or willingness to obtain CCM within 18 months of hire.
