Primary Care Physician (MD/DO)

Location: Crystal Clinic, 5200 Douglas Dr N, Minneapolis, MN 55429

Compensation: Competitive base salary, quarterly quality bonus eligibility, PTO, and CME stipend

hirify.global provides specialized primary care for women aged 65 and older through clinics across the Minneapolis–St. Paul metro area.

What you will do

• Evaluate and manage acute and chronic conditions, including diabetes, cardiovascular disease, menopause, thyroid disease, osteoporosis, and weight management.
• Develop individualized care plans and manage complex patient care longitudinally.
• Manage multiple medications, including appropriate medication reduction for poly-pharmacy.
• Provide empathetic patient education, counseling, health maintenance, and disease prevention guidance.
• Conduct physical and gynecological/well-woman examinations for older patients.
• Collaborate with the interdisciplinary care team to improve population health, coding compliance, and HEDIS quality metrics.

Requirements

• Licensed MD or DO with board certification and MBPS recognition, such as ABFM or ABIM.
• At least 3–5 years of experience managing complex disease longitudinally.
• Experience in geriatric medicine, particularly women's health, and in developing individualized care plans.
• Eligibility and willingness to obtain a Minnesota MD or DO license and DEA license, plus an NPI number.
• Proof of US citizenship or eligibility to work in the US with a permanent resident card.
• Valid Minnesota driver's license, Medicare and private payer credentialing eligibility, and successful completion of a background check.

Nice to have

• Clinical procedure experience within scope of practice.
• Value-based care experience or training.
• Experience in a startup or new clinic environment.
• Strong EMR skills, preferably Athena, and Microsoft Suite experience.
• Experience mentoring, precepting, or supervising provider staff.
