Location: 2260 S Xanadu Way, Aurora, Colorado, USA; Thursday–Sunday schedule

Salary: $75,000–$76,500 per year, based on education and experience

What you will do

  • Supervise, coach, evaluate, and develop two Peer Support Specialists.
  • Maintain a direct service caseload, including resident engagement, care coordination, crisis response, housing stabilization, and Medicaid-reimbursable services.
  • Conduct assessments, create individualized service plans, coordinate resources, and support continuity of care.
  • Monitor documentation, billing, productivity, service utilization, resident engagement, housing retention, and program compliance.
  • Coordinate supportive services with property management, behavioral health providers, assisted housing staff, and community partners.
  • Develop service procedures, coverage plans, escalation processes, and emergency response protocols.

Requirements

  • Master’s degree in Social Work, Counseling, Psychology, Public Health, Human Services, or a related field.
  • Colorado licensure required, including LCSW, LSW, LPC, LAC, or equivalent.
  • At least three years of experience in case management, supportive housing, behavioral health, homeless services, or related human services.
  • Knowledge of Medicaid billing, documentation, quality assurance, Permanent Supportive Housing, strengths-based care, trauma-informed care, and recovery-oriented practices.
  • Working knowledge of HIPAA, 42 CFR Part 2, confidentiality, information-sharing requirements, and applicable behavioral health and Medicaid regulations.
  • Strong skills in crisis intervention, conflict resolution, de-escalation, motivational interviewing, resident engagement, and cross-system collaboration.

Nice to have

  • At least one year of supervisory, team lead, or program coordination experience.
  • Experience supporting people experiencing homelessness, behavioral health conditions, substance use disorders, complex trauma, or moderate-to-high acuity needs.
