Location: Must reside in the Central region of Virginia, including Franklin City, Southampton, Sussex, Dinwiddie, or surrounding counties. This is a field-based remote role requiring 50–75% travel to members’ homes, nursing facilities, and provider locations, with occasional onsite meetings in Glen Allen, Virginia. Work hours are Monday–Friday, 8:00am–5:00pm EST.
Salary: $53,700–$72,600 per year.

What you will do
- Assess members’ goals, needs, barriers, resources, and service requirements through telephonic and face-to-face interactions.
- Develop and update Individual Service Plans with members, caregivers, primary care providers, and interdisciplinary care teams.
- Coordinate long-term services, home- and community-based services, behavioral health support, housing, education, and other community resources.
- Support members in nursing facilities, including advocating for care and assessing readiness to return to the community.
- Help members maintain Medicaid eligibility and enter annual Level of Care assessments into the state portal.
- Build partnerships with providers, payer organizations, facility staff, and community agencies to improve person-centered outcomes.
Requirements
- Residence in Central Virginia is required, specifically Franklin City, Southampton, Sussex, Dinwiddie, or surrounding counties.
- Bachelor’s degree in health or human services, or an active LPN license in Virginia without disciplinary action.
- At least two years of healthcare and/or case management experience, including one year working directly with individuals with chronic medical conditions and high social needs.
- Intermediate to advanced computer skills, including Microsoft Word, Excel, Outlook, electronic medical records, and multiple information systems.
- Strong communication, interpersonal, critical-thinking, organizational, and problem-solving skills.
- Valid state driver’s license and personal vehicle liability insurance are required. The role also requires TB screening and travel to members’ homes and facilities.
Nice to have
- Nursing home diversion or long-term care case management experience.
- Experience with Medicare and Medicaid recipients, waiver programs, complex health populations, health promotion, coaching, or wellness.
- Knowledge of community health and social service agencies.
- Bilingual or multilingual ability, especially English/Spanish, Arabic, Vietnamese, Amharic, or Urdu.
