Field Care Manager Behavioral Health

Location: Remote work from South Carolina with mandatory member home or facility visits; travel up to 25% of the time. Applicants must reside and perform work in South Carolina. Upstate or Midlands location is preferred. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ behavioral, physical, environmental, and psychosocial health needs.
• Develop care plans and monitor progress toward desired health outcomes.
• Connect members and families with appropriate healthcare and community resources.
• Identify and resolve barriers that hinder access to care and wellbeing.
• Use behavioral change, coaching, health promotion, and wellness strategies.
• Document care activities and collaborate with internal partners and members using electronic information systems.

Requirements

• Active, unrestricted South Carolina Licensed Masters Social Worker (LMSW) license or active, unrestricted compact Registered Nurse (RN) license.
• At least 2 years of experience as a behavioral health professional.
• Residence and work in South Carolina, with willingness to travel to member locations up to 25% of the time.
• A private home office with a separate room and lockable door is required to protect member PHI and HIPAA information.
• Experience with behavioral change, health promotion, coaching, and wellness.
• Proficiency with electronic information systems and Microsoft Word, Outlook, and Excel.

Nice to have

• Experience supporting patients by telephone.
• Experience in Community Mental Health, crisis response, Assertive Community Treatment, or substance use disorder treatment.
• Experience working with both children and adults.
• Residence in the Upstate or Midlands area of South Carolina.
