What you will do

• Provide skilled nursing visits in patients’ homes under physician-directed plans of treatment.
• Complete initial and ongoing patient assessments using OASIS data elements.
• Formulate, implement, and evaluate individualized plans of care with patients, caregivers, and physicians.
• Monitor patient conditions, communicate unusual reactions or changes to physicians and supervisors, and provide comfort and safety.
• Document observations, treatments, services, and patient outcomes according to agency policies.
• Supervise Licensed Practical Nurses and Home Health Aides as assigned and participate in care conferences and team meetings.

Requirements

• Graduate of an approved school of nursing.
• Current nursing license in the State of Texas.
• At least one year of current experience in a healthcare community.
• Excellent verbal communication skills.
• Home health experience is preferred.
