What you will do

• Conduct in-home clinical assessments and support patients and caregivers in starting services.
• Develop, implement, and update individualized plans of care based on patient needs, goals, and clinical condition.
• Perform scheduled home visits and regular clinical follow-up to maintain ongoing relationships.
• Use clinical judgment to identify changes in condition, safety concerns, and barriers to successful care.
• Coordinate care with physicians, caregivers, internal clinical teams, and community resources.
• Educate and coach patients and caregivers; review caregiver documentation and respond to clinical changes.
• Maintain accurate clinical documentation and ensure care delivery meets regulatory requirements and company standards.

Requirements

• Graduate of an accredited nursing program.
• Active and unrestricted Registered Nurse license in the State of Colorado.
• Strong clinical assessment, communication, and documentation skills.
• Compassionate, patient-centered approach and ability to communicate effectively with patients, families, caregivers, and physicians.
• Ability to work independently while staying closely connected to a collaborative clinical team.
• Ability to travel by car within an assigned geographic area; valid driver’s license and active auto insurance.
