What you will do

• Conduct initial and ongoing assessments of patient needs including medical, mental health, and social support requirements.
• Develop and implement comprehensive, patient-centered care plans in collaboration with medical providers.
• Coordinate services with pharmacies, insurance companies, and family caregivers to ensure continuity of care.
• Provide direct service to a caseload of chronically ill patients, facilitating referrals and addressing access issues.
• Perform crisis intervention and maintain detailed documentation in accordance with Health Home standards.
• Conduct regular home visits to monitor patient progress and ensure care plan goals are met.

Requirements

• Associate's degree required.
• Minimum 2 years of professional Care Management experience.
• Must be able to travel to and work in both the Bronx and Manhattan, NY.
• Strong working knowledge of local community resources and healthcare systems.
• Demonstrated ability to manage high-volume caseloads and handle crisis situations.
• Proficiency in data management systems such as Salesforce.
