What you will do

• Conduct virtual adult and pediatric wellness visits and health assessments directly for patients in their homes.
• Perform thorough health risk assessments, medication reviews, and cognitive screenings.
• Partner with in-home clinical team members (Sprinters) to evaluate vital signs and arrange blood draws.
• Provide comprehensive care coordination, including preventive interventions and referrals to specialists.
• Document findings and maintain patient records using HIPAA-secure messaging and charting tools.

Requirements

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in California
• Credentialed Medicaid provider for the state of California (Medi-Cal)
• Ability to pass a national background check and clinical license search.
• Proficiency with laptops, mobile devices, and secure messaging apps for care coordination.
• Strong written and verbal communication skills.
