What you will do

• Triage and provide clinical support for incoming patient communications.
• Conduct phone assessments of clinical symptoms to determine the most appropriate care path.
• Assist in the development of systems and processes for virtual triage and urgent care.
• Collaborate with multidisciplinary clinical care teams to ensure excellent patient outcomes.

Requirements

• Licensed RN with 2+ years of experience in clinical triage, preferably in ER or primary care.
• Active Nurse License Compact (NLC) required.
• Strong knowledge of chronic disease states and complex medical conditions.
• Exceptional ability to complete clinical phone assessments and communicate urgent needs.
• Proficiency in written communication and use of technology.

Nice to have

• Active RN license(s) in NY, NV, MI, MN, OR, CA, MA, CT, or IL.
• Experience in high acuity clinical environments such as the ED or ICU.
• Experience in geriatrics.
• Bilingual proficiency in English and Spanish.
