Salary: $26.82 – $32.37 per hour

hirify.global (AFS) provides essential mental health and social services to children, youth, and families within the Algoma District.

What you will do

• Manage the intake process for youth and families, utilizing tools like the Strengths and Difficulties Questionnaire (SDQ) to determine eligibility and priority.
• Provide intensive case management and system navigation for high-risk children with complex, multi-sectoral needs.
• Coordinate cross-sectorial service resolution in collaboration with local and regional providers.
• Conduct single therapy sessions and other clinical treatment services as designated.
• Develop initial service plans and maintain accurate client records in the EMHWare database.
• Perform outreach and travel across the Algoma District to deliver intake functions.

Requirements

• Bachelor's degree in Social Work (preferred, 2+ years exp) or Psychology/Social Services (preferred, 3+ years exp), or a relevant College Diploma.
• Proven experience in intensive case management for children's mental health, developmental, or rehabilitation services.
• Valid Ontario Driver’s License (Class G or G2) and access to a personal vehicle.
• Clear Criminal Records Check and full immunization.
• Proficiency in Microsoft Suite and experience with information databases (e.g., EMHWare).
• Availability to work flexible hours, including evenings and occasional weekends.
