Outreach Representative (Healthcare)

What you will do

• Act as a primary liaison between members, the Health Plan, and state/federal regulators.
• Conduct telephonic outreach to assess member needs and provide appointment scheduling assistance.
• Educate members on their rights, responsibilities, and the importance of preventive health services.
• Recruit and enroll members into Disease Management Programs and refer them for Care Management.
• Coordinate access to preventive health services by collaborating with providers and community agencies.
• Maintain accurate records of all member and provider contacts in corporate computer systems.

Requirements

• High School Diploma or GED required.
• 1 year of experience in Community Outreach or Customer Service.
• Must be located in Pennsylvania.
• Strong active listening skills and empathy when interacting with diverse populations.
• Ability to work independently and effectively within a team environment.
• Proven time management skills and the ability to handle multiple tasks simultaneously.

Nice to have

• Associate’s Degree.
• Familiarity with healthcare or insurance industry.
• Experience working specifically with Medicaid and/or Medicare populations.
