What you will do

• Conduct virtual primary care visits via telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions.
• Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and develop individualized care plans.
• Order and interpret lab tests, imaging, and other diagnostics; coordinate follow-up care as needed.
• Manage medication prescriptions, including controlled substances, titrations, and refills, ensuring safety and adherence.
• Provide health coaching, lifestyle guidance, and patient education to support proactive care.
• Coordinate referrals and in-person services (labs, imaging, specialist referrals) and participate in clinical case reviews and team huddles.

Requirements

• Licensing: Active APRN (NP/PA) license in good standing in AL, AZ, AR, CO, FL, GA, HI, IL, IN, KY, MS, MO, NC, OH, PA, SC, TN, or TX; must be willing to obtain additional licenses and a DEA certification within the first 90 days (support and reimbursement provided).
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably including telehealth/virtual care.
• Experience managing chronic conditions, preventive care, and acute illnesses via telehealth.
• Comfort with digital documentation and telemedicine platforms (EHR and virtual care workflows preferred).
• Active BLS certification at time of hire; minimum 6 months of experience working as a Nurse Practitioner in the D2Me program required.
• Shift times may vary to align with standard business hours (9:00 AM–5:00 PM) in the patient’s local time zone; flexibility required.
