What you will do

• Deliver coordinated, patient-centered virtual care management via telephone or video.
• Create impactful care plans in partnership with members and a diverse clinical team.
• Help members navigate complex medical conditions, treatment pathways, and healthcare benefits.
• Partner with local providers to ensure coordinated, longitudinal care.
• Provide compassionate follow-up during acute healthcare episodes like hospitalizations.
• Coordinate resources to address both clinical and social determinants of health.

Requirements

• Bachelor of Science in Nursing (BSN) required.
• Active California Nursing License and active Compact RN license in good standing.
• Must reside in a compact NLC state.
• 5+ years of nursing experience and 2+ years in care management.
• Must have current CCM Certification or be eligible for the exam by August 2026.
• Ability to work until 6pm PST.
