What you will do

• Review member medical histories, lab results, and medications to determine program eligibility.
• Execute intake protocols and clinical assessment decision trees to guide member enrollment.
• Collaborate with cross-functional teams, including nurses, health coaches, and providers, to ensure a seamless member experience.
• Provide initial clinician reviews and escalate complex cases to the Twin provider for further assessment.
• Maintain precise clinical documentation and records in accordance with federal law and company policy.
• Utilize nursing processes to triage member alerts and inquiries efficiently.

Requirements

• Active and unencumbered compact RN license (additional state licenses preferred).
• BSN or MSN from an accredited school of nursing.
• Minimum 4 years of nursing experience across various healthcare systems (hospitals, ambulatory care, or payor systems).
• Experience in triage, intake, case/care management, or population health.
• Proficiency in motivational interviewing and facilitating behavior change.
• Ability to manage large caseloads and multitask using various electronic documentation systems in a fast-paced environment.

• Bilingual fluency in Spanish.
