Location: Remote (USA). Requires working hours of Monday-Friday 8:30am-5pm MST or PST preferred (EST and CST also considered).

Salary: $120K-155K base range (Total comp $130K-170K)

What you will do

• Conduct virtual consultations for patients with hypertension, including diagnostics and developing individualized treatment plans.
• Proactively initiate and manage statin therapy to mitigate cardiovascular disease risk in patients with diabetes.
• Collaborate with an interdisciplinary team including primary care providers, pharmacists, social workers, and health coaches.
• Serve as a clinical advisor and provide escalation support for specialty clinic staff.
• Utilize and provide feedback on a proprietary electronic health information platform to improve clinical interfaces.
• Maintain accurate patient medical records in compliance with legal and ethical telehealth regulations.

Requirements

• Active and clear RN and APRN or Physician Assistant license.
• Fully bilingual in Spanish and English.
• 5+ years in outpatient clinical practice, with 2+ years concentrated in primary care or relevant subspecialties (cardiology, nephrology, endocrinology).
• Willingness to obtain and maintain multiple US state licenses.
• Strong clinical foundation in primary and secondary prevention of atherosclerotic cardiovascular disease (ASCVD).

Nice to have

• Familiarity with AI-assisted clinical workflows.
• Proficiency in using telehealth technology and electronic health records (EHR).
• Virtual care experience and an understanding of managed care (STARS/HEDIS measures).
