TL;DR

RN Nurse Navigator (Medical): Coordinating patient flow and clinical care for adult patients within specialized disease states with an accent on multidisciplinary collaboration and community outreach. Location: Hybrid. Must live within 50 miles of the Greater Pittsburgh or Greater Erie region (PA)

What you will do

• Serve as the primary point of contact for referring physicians, patients, caregivers, and families.
• Coordinate all aspects of patient flow, including appointments and access to supportive care services.
• Oversee community outreach and establish relationships with health plans, physicians, and social services.
• Educate patients on diagnoses, treatment options, clinical trials, and available resources.
• Collaborate with High-Risk Care Teams to ensure safe handoffs and reduce patient re-admissions.
• Analyze program data and outcomes to identify and close gaps in patient care delivery.

Requirements

• Current State of PA RN licensure or multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
• 1-3 years of nursing experience in the required specialty.
• CPR certification from the American Heart Association.
• Act 34 Criminal Background, Act 33 Child Abuse, and Act 73 FBI Fingerprinting clearances.

Nice to have

• Bachelor's Degree in Nursing (BSN).
• Professional certification in the area of expertise.
• Valid PA driver's license.
