Salary: $120,000–$150,000/year (Total compensation up to $170,000 including performance bonuses).

What you will do

• Conduct comprehensive post-discharge assessments and perform full medication reconciliations.
• Perform virtual urgent-care visits to diagnose and manage new or worsening symptoms in medically complex patients.
• Manage the clinical trajectory over a 30-day window, adjusting whole-person chronic-care plans.
• Collaborate with an interdisciplinary team of RNs, social workers, and physicians during rounds.
• Contribute to the development of the TOC program and provide feedback to improve the internal EHR.

Requirements

• Proficiency in English and Spanish is required.
• 5+ years of experience managing geriatric populations with multiple complex comorbidities.
• Active and in-good-standing RN/APRN or Physician Assistant license.
• Licensure in FL, TX, AZ, OH, and/or GA required to start.
• Must be able to work a full-time schedule (8:30 AM – 5:00 PM) within CST, MST, or PST hours.
• Strong independent clinical decision-making skills in a virtual setting.

Nice to have

• Experience in internal medicine, hospital medicine, urgent care, or palliative care.
• Experience performing visits over telehealth video platforms.
• Experience working with frail or vulnerable populations in skilled nursing facilities (SNF).
