• Manage an assigned caseload of low-risk members through telephonic outreach and follow-up.
• Coordinate referrals for primary, specialty, and behavioral health services while tracking completion.
• Build trusting, culturally responsive relationships with members to support enrollment and engagement.
• Collaborate with an interdisciplinary team including RNs, Behavioral Health Managers, and Community Health Workers.
• Maintain accurate documentation of member interactions and care coordination activities in EHR systems.
• Assist members with appointment scheduling, transportation logistics, and durable medical equipment requests.

• Fluency in English and Spanish (verbal and written) is required.
• Must be authorized to work in the United States.
• High School Diploma or GED required; Associate’s or Bachelor’s degree preferred.
• 1–3 years of experience in healthcare, care coordination, or patient navigation.
• Strong organizational skills and ability to manage multiple technology platforms simultaneously.
• Willingness to travel occasionally for team meetings or community events.
