What you will do

• Enter and manage patient care coordination requests within Salesforce Health Cloud.
• Update patient-specific tasks and lists based on daily census reports.
• Coordinate with outpatient treatment providers to confirm appointment attendance.
• Collaborate with hospital discharge planners to manage notifications and appointment details.
• Maintain schedules for clinical staff and support process automation initiatives.
• Work closely with the clinical team to enhance overall quality of care.

Requirements

• Must be based in the United States
• 2 years of experience in outpatient clinical office roles (brick-and-mortar or virtual).
• Proficiency in Microsoft Word, Excel, and Outlook.
• Experience with technology solutions supporting clinical workflows.
• Strong oral and written communication skills.
• Ability to navigate ambiguity and work in a fast-paced environment.
