What you will do

• Obtain and document vital signs in the EHR during patient visits.
• Perform venipuncture and capillary blood collection, prepare specimens for lab analysis, and assist with minor procedures as needed.
• Administer immunizations/injections as able per physician orders and maintain accurate vaccine records.
• Educate patients on treatment plans, medications, and follow-up care; complete physical and social screenings as assigned.
• Manage empaneled patient chart readiness, run-the-list care coordination, and communicate with patients by text/video/phone for scheduling and reactive needs.
• Support population health gap closures and coordinate prior authorizations, referrals, and medical record requests with local providers and hospitals.

Requirements

• At least 2 years of clinical experience as a Licensed Practical Nurse.
• Phlebotomy experience is required.
• Reliable access to personal transportation.
• Strong communication skills and ability to explain complex ideas simply.
• Strong people skills, organizational skills, and ability to manage multiple priorities.
• Ability to travel to see patients within Aroostook County and be available for field/urgent deployment within the market region.
