Location: On-site in Phoenix, AZ (Physical presence mandatory 10:00 AM to 2:00 PM); path to hybrid schedule available as the program scales.

Salary: $22.00–$24.00 per hour

What you will do

• Act as the primary on-site point of contact for unannounced MAC field inspections.
• Ensure facility readiness, verify signage, and maintain strict HIPAA-compliant records storage.
• Facilitate patient enrollment into the CCM portal and manage daily outbound engagement.
• Collect and document patient health information and evolving symptoms.
• Provide evidence-based health support resources, including exercise and meditation content.
• Maintain the Readiness Binder containing licenses, lease, and 855B applications.

Requirements

• Active LPN or LVN license in good standing in the state of Arizona.
• Minimum 3 years of experience as an LPN or LVN in clinical or remote care settings.
• Strict availability for on-site presence in Phoenix, AZ during mandatory inspection windows.
• Proficiency with Zoom, Slack, and online patient enrollment platforms.
• Personal laptop with working microphone, camera, and reliable internet connection.
• Demonstrated commitment to HIPAA compliance and patient privacy.

Nice to have

• Experience in Orthopedics, Physical and Rehabilitation Medicine, Sports Medicine, or Neurology.
