What you will do

• Complete structured assessments of medical, bio-psychosocial, and self-management support needs.
• Act as an advocate and liaison between patients, families, hospitals, and community resources.
• Maintain accurate documentation to promote team awareness and ensure patient safety.
• Assist patients with registration, billing, and navigating healthcare system barriers.
• Collaborate with primary care teams to establish and update shared care plans.
• Provide clinical workflow oversight and support for patient goal setting and action planning.

Requirements

• Must be based in Pennsylvania
• Graduate of a Medical Assistant or LPN program, or equivalent education/experience.
• Minimum of 5 years of experience in a Medical Assistant or related role.
• Strong communication, problem-solving, and empathy skills.
• Ability to comply with HIPAA and data security regulations.
