What you will do
- Conduct face-to-face needs assessments in member homes and nursing facilities.
- Coordinate care across physical, behavioral, and long-term service delivery systems.
- Develop and implement individualized care plans to meet member goals.
- Facilitate transitions between hospital, home, and community-based care settings.
- Educate members and caregivers on available benefits, resources, and long-term care options.
- Document all case management interventions in the electronic health record system.
Requirements
- Must be based in Delaware with reliable transportation for frequent travel.
- Bachelor's degree in Social Work/Health/Human Services with 3 years of experience OR Master's degree with 1 year of experience.
- Alternatively, current RN or LPN licensure with 2 years of relevant experience.
- Proficiency in PC-based word processing and database documentation.
- Ability to work flexible hours to meet member needs.
- Compliance with all HIPAA and state privacy regulations.
Nice to have
- Certified Case Manager (CCM) certification.
- Experience with Medicare, Medicaid, or managed care.
- Background in geriatric special needs, behavioral health, or developmental disabilities.
- Licensed Social Worker (LBSW, LMSW, or LCSW).
