Salary: $52 - $58 per hour

What you will do
- Manage patient plans of care from initial contact through to discharge, ensuring functional progress and safety.
- Perform and document evaluations and re-evaluations using appropriate treatment modalities and protocols.
- Collaborate consistently with treating PTs and PTAs to maintain continuity of care.
- Maintain detailed patient records in the electronic medical record (EMR) system.
- Provide patient and family education using methods appropriate to their understanding.
- Serve as a clinical instructor and mentor for interns and new hires.
Requirements
- Bachelor’s Degree in Physical Therapy.
- Licensed Physical Therapist in the State of Illinois.
- BLS certification from the American Heart Association upon hire.

- Masters or Doctoral Degree in Physical Therapy.
