What you will do

• Manage patient plans of care from initial contact through to discharge, ensuring functional progress and safety.
• Perform and document evaluations and re-evaluations using appropriate treatment modalities and protocols.
• Collaborate consistently with treating PTs and PTAs to maintain continuity of care.
• Maintain detailed patient records in the electronic medical record (EMR) system.
• Provide patient and family education using methods appropriate to their understanding.
• Serve as a clinical instructor and mentor for interns and new hires.

Requirements

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in the State of Illinois.
• BLS certification from the American Heart Association upon hire.

Nice to have

• Masters or Doctoral Degree in Physical Therapy.
