Salary: $29.00 - $31.00 per hour

What you will do

• Conduct virtual and in-person outreach to patients for registration and initial intake.
• Provide care navigation, appointment logistics, and support for prescriptions and labs.
• Assess social determinants of health (SDOH) and enroll patients in appropriate support programs.
• Collaborate with a multidisciplinary care team to develop and execute comprehensive patient care plans.
• Organize community events and facilitate group health education sessions to build trust.
• Document all patient and care team interactions within EHR systems and other internal tools.

Requirements

• Fluency (verbal and written) in English and Spanish.
• High school diploma or GED.
• CHW certification OR 2,000+ hours of CHW work in the past three years.
• 1-3 years of healthcare experience and 2-5 years of community advocacy or organizing.
• Must be based in or able to travel to other communities around Concord, CA.
• Full COVID-19 vaccination required.

Nice to have

• Knowledge of Latino customs and cultural norms.
• Experience documenting in an EHR and familiarity with Google Workspace.
• Training in motivational interviewing.
• Strong ties and recognition within the local Latino community.
