What you will do

• Represent the organization at community events, health fairs, and partner locations.
• Conduct face-to-face outreach to increase awareness of programs and services.
• Identify and engage individuals who may benefit from organizational support.
• Develop and maintain relationships with healthcare providers, shelters, and community organizations.
• Educate community members about Medicaid-related programs and social determinants of health.
• Document outreach efforts, referrals, and community contacts in CRM systems.

Requirements

• Must be based in The Bronx
• 1–3 years of experience in community outreach, healthcare, sales, or social services.
• Strong communication and interpersonal skills for connecting with diverse populations.
• Ability to work independently and manage a flexible schedule in the field.
• Comfortable attending events and conducting face-to-face outreach.
• Valid driver's license and reliable transportation.

Nice to have

• Associate's or bachelor's degree.
• Experience using CRM systems and Microsoft Office applications.
