What you will do

• Reconcile ERA/EOB payments and resolve underpayments or missing remittances.
• Maintain AR aging and communicate directly with payer representatives to resolve outstanding balance disputes.
• Perform pre-submission claim scrubbing for demographics, eligibility, and coding accuracy.
• Resolve claim rejections from RCM platforms and payer portals, distinguishing between clearinghouse and payer-level fixes.
• Coordinate with the RCM platform team to manage shared AR workqueues.
• Track rejection patterns and provide recommendations for upstream workflow improvements.

Requirements

• 3+ years of medical billing experience in AR reconciliation, claim submission, or rejection management.
• Must be based in the United States.
• Proficiency with 837 claim files, 277 rejection reports, and ERA/835 remittance files.
• Experience with Medicaid managed care and Medicare Advantage payer requirements.
• Ability to manage and reprioritize multiple high-volume work queues simultaneously.

Nice to have

• Experience resolving capitation or encounter-based payment methodology disputes.
• Prior experience working in a lean or startup RCM environment.
• Exposure to value-based care or risk-adjusted billing models.
• Experience with CMS-1500 and UB-04 across institutional and professional billing.
