Patient Engagement Associate II (Healthcare)

TL;DR

Location: Remote (US), but must be able to regularly travel to Houston, TX and Kansas City, MO

Salary: $24–$26 per hour

A tech-enabled, pediatrician-led medical group providing virtual and in-home care for children with special healthcare needs.

What you will do

• Conduct research on attributed patients to identify healthcare needs and potential barriers to enrollment.
• Perform telephonic outreach to caregivers and healthcare professionals to enroll children in the program.
• Handle inbound phone calls from caregivers, health plan service coordinators, and hospital discharge planners.
• Develop deep knowledge of the Supplemental Security Income (SSI) program to assist eligible caregivers.
• Collaborate with Provider Engagement Specialists to perform on-site outreach at PCP offices and residential facilities.

Requirements

• 2+ years of experience in healthcare care coordination, community health, patient engagement, customer service, or sales.
• Proven success working within a centralized contact center environment.
• Strong communication and active listening skills to tailor value propositions and handle complex objections.
• Must have access to reliable transportation and internet.
• Ability to work autonomously and pivot quickly within a startup environment.

Nice to have

• Bilingual Spanish proficiency.
• Prior experience with CRM platforms.
