Outreach Representative (Healthcare)

What you will do

• Conduct telephonic outreach to assess member needs and provide appointment scheduling assistance.
• Educate members on their rights, responsibilities, and the importance of targeted preventive health services.
• Enroll members into Disease Management Programs and refer them to Care Management interventions.
• Coordinate access to preventive health services by contacting members, providers, and community agencies.
• Track all member and provider contacts in computer systems for data collection and record keeping.
• Participate in the development of innovative and measurable outreach programs for at-risk members.

Requirements

• High School Diploma or GED.
• 1 year of experience in Community Outreach or Customer Service.
• Must be based in Delaware.
• Strong active listening skills and the ability to act sensitively toward diverse people.
• Ability to prioritize and handle multiple tasks simultaneously with sound judgment.

Nice to have

• Associate’s Degree.
• Familiarity with healthcare and/or insurance industries.
• Experience working with Medicaid and/or Medicare populations.
