What you will do

• Collaborate with interdisciplinary teams, including nurses and social workers, to deliver comprehensive virtual care services.
• Conduct virtual nutritional assessments for patients with chronic kidney disease (CKD) and other co-morbid conditions.
• Develop personalized, evidence-based nutrition care plans and calculate requirements tailored to various CKD stages.
• Educate patients and caregivers on kidney-related dietary and lifestyle adjustments.
• Maintain thorough and timely documentation of all patient interactions in compliance with regulatory requirements.
• Practice within the registered dietitian scope and make necessary interdisciplinary referrals.

Requirements

• Bachelor’s Degree in Nutrition, Dietetics, or a related field from an accredited institution.
• Active unrestricted Registered Dietitian certification issued by the Commission on Dietetic Registration.
• Active state dietitian licensure or registration; willingness to obtain additional state licensures as needed.
• 2+ years of experience in delivering care to patients with kidney disease or other chronic diseases.
• Strong knowledge of healthcare regulations, including HIPAA.
• Proficiency in using virtual care platforms and experience in patient counseling.
