What you will do

• Conduct telehealth video visits and occasional home-based visits to members.
• Perform comprehensive assessment visits (CAVs), including diagnosis and medication reviews.
• Execute diagnostic tests, including mobile screenings and fingerstick tests.
• Collaborate with PCPs, specialists, pharmacy, and social work to coordinate holistic care plans.
• Utilize and provide feedback to improve the internal electronic health information system.
• Support clinical nurses via an infrequent weekend on-call component in certain geographies.

Requirements

• Active and clear RN and APRN license in the specified market (SC, GA, or OR) and APRN certification.
• Active BLS certification required at time of hire.
• 3+ years of experience in outpatient clinical practice.
• Experience in primary care, family medicine, geriatrics, and/or palliative care.
• Experience performing Medicare annual wellness visits or in-home comprehensive visits.
• Must be based in the USA.

Nice to have

• Bilingual proficiency in Spanish or Haitian Creole.
• Prior home-based practice experience.
• Understanding of managed care, including STARS/HEDIS measures and clinical comorbidities.
• Multi-state licensure.
