What you will do

• Deliver diabetes education and support to improve adherence and health outcomes.
• Establish rapport via telehealth platforms with diverse patients.
• Manage medications, including adjustments within protocol and provider recommendations.
• Monitor remote devices for data on medication, safety, and goals.
• Drive behavior change using motivational interviewing.
• Present case studies in team meetings and meet scheduling/quality metrics.

Requirements

• Must reside in the United States and have a home office with high-speed internet.
• Current CDCES certification from NCBDE.
• Experience with motivational interviewing for behavior change.
• Knowledge of diabetes technologies: CGM, AID systems, CSII, connected insulin pens.
• Diabetes medication management experience, including adjustments.
• Comfort with EMR, Microsoft Office, scripting, and advanced tech.
• Flexible schedule, minimum 20 hours/week commitment; no full-time employment elsewhere.
