Salary: $73,700 - $103,180 + equity + benefits

What you will do

• Deliver coordinated, patient-centered virtual care management by telephone or video to improve health outcomes.
• Create impactful care plans with members and diverse care team to achieve goals.
• Guide members through complex medical conditions, treatment pathways, benefits, and healthcare system.
• Partner with local providers for coordinated care and provide compassionate longitudinal follow-up.
• Assist during acute episodes like hospitalizations and rehab, coordinating case management for members and families.
• Coordinate clinical and social resources to holistically address members' needs.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM certification.
• 5+ years nursing experience; 2+ years in care/case/disease management, preferably in health plan or TPA.
• 2+ years in Behavioral Health and/or substance use field.
• Must reside in a compact NLC state with Active Compact RN license in good standing.
• Work until 9-6PM PST; comfortable with multiple medical conditions across age ranges.
• Experience working remotely and using multiple computer/medical record systems.

Nice to have

• Active California Nursing License.
• Spanish speaking.
