What you will do

• Implement the Collaborative Care Model (CoCM) adhering to University of Washington AIMS Center guidelines.
• Coordinate mental and physical health care for assigned patients in collaboration with medical providers and other professionals.
• Screen and assess patients for mental health disorders and facilitate follow-up care.
• Deliver evidence-based behavioral interventions, including motivational interviewing and problem-solving treatments.
• Collaborate with psychiatric consultants and licensed neuropsychologists to optimize patient treatment plans.
• Maintain accurate records in the EHR system and develop relapse prevention plans for patients.

Requirements

• Active LCSW (or similar) licensure.
• Minimum of 2 years’ experience in direct case or care management.
• Background in ALS, Parkinson’s, Dementia, or other chronic neurodegenerative disease care.
• Must be based in the specified US regions: TX, NM, OH, IL, FL, AZ, or WA.
• Familiarity with insurance processes, medical billing/coding, and reviewing medical records.
• Experience working within clinical trials and ability to thrive in a startup environment.
