TL;DR

Location: Must be located within the lower 48 United States due to contractual limitations

What you will do

  • Engage members via phone, text, and email to identify health/social needs, discuss goals, and connect to resources including nurses, providers, transportation, financial aid, and emotional support.
  • Assess urgency, determine root causes, and establish next steps using evidence-based assessments and problem-solving.
  • Serve as primary point of contact, collaborating with interdisciplinary teams for holistic support.
  • Handle inbound/outbound calls in a high-volume phone queue, managing documentation and coordination tasks.
  • Follow policies, meet productivity/quality metrics, and participate in coaching.

Requirements

  • Located within the lower 48 United States with quiet, secure remote workspace and reliable internet.
  • At least 2 years in patient-facing roles like care coordination, healthcare navigation, non-clinical case management, or community health work.
  • Experience in fast-paced, high-volume, metrics-driven environments (e.g., contact centers).
  • Strong communication, empathy, rapport-building, and ability to identify unstated needs.
  • Comfort with technology: Google Suite, Slack, EHR, video chatting.
  • Organized, adaptable, with proven success in feedback-oriented settings.
