What you will do

• Engage eligible patients through telephonic, written, and digital outreach methods.
• Leverage available data from EMR to learn more about potential patients and inform outreach processes.
• Complete the intake and consent process for eligible patients, and transitioning them to a Care Coordinator upon full enrollment.
• Connect authentically with patients and their families in a non-judgmental, respectful, and empathic manner and striving to meet their needs.
• Work daily within EMR to collect data, organize information, track tasks, and communicate with staff, patients, and family members.

Requirements

• You have 0-2 years of customer service or sales experience with a successful track record to share for that time period.
• You have a passion for working within the community you are a part of, or have been a part of in the past.
• You have dynamic interpersonal skills and a strong desire to improve the health of individuals and whole communities.
• You have a metrics-driven approach to your work and are willing to do whatever it takes to meet quotas for the Outreach team.
• You are comfortable with high-volume outbound calls as well as creative outreach.
• You are an independent self-starter who responds well to change and shifting priorities.
