Location: Remote (Must be based in the United States)

Salary: $27 - $30 an hour

What you will do

• Establish and manage compassionate relationships with patients and caregivers, serving as the primary point of contact.
• Execute patient care plans, providing ongoing support and coordinating connections to neurology and genetic experts.
• Provide basic health education, including information about diagnosis, treatment, care options, and research opportunities.
• Develop and adhere to standardized processes, including operating procedures, quality assurance, and documentation.
• Maintain positive relationships within the multidisciplinary Care Coordination and broader hirify.global team.
• Maintain working knowledge of research development and other trends and advances in neurodegenerative diagnoses, treatment, and care.

Requirements

• Bachelor’s degree in a related field.
• 2+ years of experience in direct case or care management in a healthcare setting.
• Comfortable using technology to support members without in-person contact.
• Excellent verbal, written, organizational, and interpersonal skills.
• Understanding of scheduling platforms and electronic health record systems.
• Proficient in collecting member clinical and demographic data and documenting appropriately.

Nice to have

• Experience with clinical care of patients with neurodegenerative diseases.
• Bilingual, with verbal and written fluency in Spanish.
