What you will do

• Conduct outbound calls, SMS, and email outreach to drive patient enrollments.
• Document and process incoming referrals accurately in internal systems.
• Coordinate with health plan case managers and external providers to track referral status.
• Manage inbound inquiries from potential patients and partners, providing guidance on available resources.
• Achieve company enrollment KPIs and assist families with scheduling appointments.
• Participate in ongoing training regarding healthcare regulations and clinical trends.

Requirements

• Must be based in the United States
• Bachelor's degree in social work, healthcare, or a related field.
• Excellent communication and interpersonal skills.
• Experience with healthcare referral systems and documentation.
• Familiarity with working with people assigned to Medicaid health plans.
• Strong organizational and time-management skills.
