Palliative Care NP Manager

Location: Remote USA

Salary: $140,000–$170,000 per year

What you will do

• Provide direct palliative care by phone and video visits for enrolled patients.
• Supervise palliative care clinical staff, including a mix of nurse practitioners, registered nurses, and social workers.
• Lead patient panel reviews and participate in weekly interdisciplinary team meetings.
• Coach direct reports, set goals, review performance, and create performance improvement plans when needed.
• Conduct clinical quality audits through note reviews and listening tours.
• Support referral management, recruitment, onboarding, process improvement, and change management.

Requirements

• At least 5 years of experience as a palliative care clinician.
• At least 2 years of experience leading frontline clinical teams with accountability for team-level outcomes.
• Demonstrated ability to champion initiatives and achieve business outcomes.
• Strong clinical and non-clinical judgment, with the ability to coach performance and improve clinical effectiveness.
• Empathetic, organized, detail-oriented, collaborative, and comfortable working in a fast-paced environment.
• Ability to work remotely from the USA.
