Nurse Practitioner (1099 / PRN) – In-Home Assessments

Location: On-site in Bushnell, Florida, United States; travel throughout Lake County, Sumter County, and Citrus County, with a maximum 55-mile radius from the assigned home location

Pay: Approximately $100 per completed assessment plus paid mileage

What you will do

• Conduct in-home Annual Wellness Visits and chronic condition risk-adjustment assessments.
• Perform focused histories, physical examinations, vital-sign checks, medication reviews, and screenings.
• Complete Hemoglobin A1C, spirometry, KED, DRE, and FIT screenings.
• Educate members on preventive care and chronic disease management.
• Document assessments in the EMR to support value-based care and risk adjustment.
• Travel independently to assigned members’ homes and honor confirmed visits.

Requirements

• Active, unencumbered Nurse Practitioner license in the state where services are provided.
• Reliable transportation and willingness to travel within the designated Florida coverage area and up to a 55-mile radius.
• Ability to work independently and use EMR systems effectively.
• Approximately 30 hours of ongoing monthly availability and consistent responsiveness during onboarding and scheduling.
• Three years of patient care experience is preferred, particularly in primary care, adult, geriatric care, or EMR-based settings.
• In-home risk-assessment experience is preferred; additional NP licensure may be required and is supported by hirify.global.
