Clinical Pharmacist

Location: London, United Kingdom; hybrid work with remote home working, travel to practices and regional meetings as required.

What you will do

• Support primary care practices with clinical tools and resources for managing patients with long-term conditions.
• Deliver clinical services in line with protocols, operating instructions, and internal systems.
• Identify opportunities for clinical reviews at practice, PCN, and ICB level.
• Build awareness of Interface Clinical Services and develop lasting relationships with practices and local NHS organisations.
• Manage a flexible diary while travelling to practices and working remotely when appropriate.

Requirements

• GPhC registration and at least two years of post-qualification experience are required.
• Primary care experience is desirable.
• PCPEP qualification is desirable.
• Full UK driving licence required.
• Willingness to travel as required and work remotely from home.
• Passion for improving patients' long-term health and a driven work ethic.
