LTSS Service Care Coordinator (Healthcare)

Location: Remote in Iowa; Monday–Friday, 8:00 AM–5:00 PM CST. The role may include on-site member visits.

Salary: $22.94–$38.79 per hour

What you will do

• Assess member needs, available resources, and appropriate care outcomes.
• Develop and monitor long-term care plans and service plans.
• Coordinate members, families, caregivers, providers, specialists, and community resources.
• Connect members with support related to housing, employment, independent living, justice, foster care, and participant direction.
• Educate members and caregivers about services, referrals, provider instructions, procedures, and healthcare benefits.
• Document care management activities, monitor outcomes, support regulatory compliance, and recommend quality improvements.

Requirements

• Bachelor’s degree required.
• At least 1 year of related experience required.
• For the Iowa Plan: 1+ year of experience serving relevant populations is required, or an RN with 6+ years of experience serving relevant populations.
• Ability to coordinate long-term care services and communicate with members, caregivers, providers, and community partners.
• Ability to perform occasional on-site visits when needed.
