Cardiology Patient Navigator (Coordinator) (Cardiology)

Location: Remote from Bogotá, Colombia; U.S. business hours (CST) required

Salary: USD 5–6 per hour

hirify.global supports a U.S.-based cardiology care team with remote patient coordination and clinical operations.

What you will do

• Manage patient scheduling, reminders, follow-up workflows, and care-plan communication.
• Conduct telephonic and remote outreach to coordinate next steps and reinforce care instructions.
• Support cardiology care pathways, including CHF management protocols.
• Track testing, appointments, remote patient monitoring workflows, and continuity of care across a caseload of 150+ patients.
• Escalate clinical concerns to providers within scope without diagnosing or prescribing.
• Maintain clinical records, support billing documentation, and coordinate insurance-related information.

Requirements

• Clinical healthcare background such as CNA, MA, RN, or an equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Care coordination or care management experience, preferably in telephonic or remote settings.
• B2+ English and B2+ Spanish proficiency required.
• Experience with EMR/EHR systems and strong patient-centered communication skills.
• Ability to work U.S. business hours (CT) and work remotely from Bogotá, Colombia.

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, or home health.
• Outpatient cardiology, home health, or telemedicine experience.
• Remote patient monitoring, medical billing, or prior authorization experience.
• Contractor experience or experience working in startup and evolving environments.
