Clinical Pharmacist (Cardiology / Value-Based Care)

Location: Remote — anywhere in the US; must be able to work during the client's business hours in CT

Rate: $60–$65 USD/hour

What you will do

• Review medication profiles for heart failure, coronary artery disease, atrial fibrillation, hypertension, CKD, and related cardiac conditions.
• Evaluate and optimize guideline-directed medical therapy using ACC/AHA guidelines and payer quality programs.
• Identify therapy gaps, contraindications, drug interactions, and adherence barriers, then recommend medication initiation, optimization, and titration.
• Conduct telephonic medication reviews and patient education on medication use, side effects, adherence, lifestyle considerations, and red-flag symptoms.
• Collaborate with RN Care Managers, Nurse Navigators, cardiologists, and advanced practice providers on care planning and interdisciplinary case reviews.
• Document recommendations and patient interactions in the care management platform or EMR, while supporting medication-related quality measures and reducing avoidable utilization.

Requirements

• PharmD and active Texas pharmacist license in good standing.
• Multi-state pharmacist licensure; the remote role is not restricted to Texas residency.
• 3–5 years of clinical experience with a strong cardiology focus and demonstrated knowledge of GDMT and cardiac medication titration.
• Experience working in a nurse-led care management model and collaborating with physicians, cardiologists, and advanced practice providers.
• Strong telephonic patient communication skills and a patient-centered communication style.
• Exceptional written English, assessed during screening, and availability during the client's CT business hours.

• Experience in heart failure or cardiology clinics.
• Experience with clinically integrated networks or value-based care models.
• Familiarity with ACC/AHA guidelines and payer quality programs.
• Experience documenting in electronic medical records or care management platforms.
• Prior remote or telehealth patient engagement experience.
