Acute Care Nurse Practitioner - Hospice, Part-time, Days

Location: Lemont, Illinois, United States; 100% inpatient

What you will do

• Provide comprehensive hospice and palliative care across inpatient and community settings.
• Conduct face-to-face hospice visits, symptom management, health assessments, and eligibility evaluations.
• Round on new general inpatient hospice admissions and participate in interdisciplinary team meetings.
• Assess patients, perform examinations, prescribe or order medications and therapies, and interpret tests within clinical privileges.
• Educate patients, families, staff, and external groups while mentoring students and serving as a clinical consultant.
• Participate in quality improvement, research, committees, and continuing education initiatives.

Requirements

• Master’s degree from an accredited nursing or physician assistant program.
• Acute care certification for APRNs, or qualifying inpatient NP experience in lieu of certification.
• First Assist certification for all APRNs.
• Current Illinois RN/APRN or physician assistant licenses and required state credentials.
• Credentialing and privileging through the hirify.global Medical Staff Office.
• Current BLS certification issued by the American Heart Association.

• Hospice, palliative care, oncology, or critical care experience.
