Housing Health Advocate (Emergency Department)

Location: 676 N. St. Clair, Chicago, IL; onsite work across emergency department, hospital, outpatient clinic, and community settings.

Salary: USD 25.33–37.99 per hour

What you will do

• Provide case management and support to patients with chronic illnesses, homelessness, and complex medical, behavioral health, and social needs.
• Serve as the hirify.global representative in the Emergency Department Transitions Hub and build therapeutic relationships with patients.
• Coordinate warm handoffs to community partner organizations providing case management, housing navigation, shelter, and supportive services.
• Develop individualized transition plans, coordinate transportation, and assist with benefits, housing assessments, documentation, and placement activities.
• Collaborate with physicians, pharmacists, social workers, therapists, psychiatrists, hospital teams, and community partners.
• Track patient outcomes, transition milestones, and clinical information while identifying barriers and supporting continuous improvement.

Requirements

• Bachelor’s degree required.
• One to two years of experience providing medical or mental health case management.
• Knowledge of housing, homelessness, healthcare, and community-based support resources.
• Ability to coordinate care, resolve conflicts, document accurately, and work with interdisciplinary teams.
• Ability to conduct patient visits in diverse communities and comply with HIPAA and patient confidentiality requirements.

Nice to have

• Bachelor’s degree in a human services field or a BSW.
• CPR certification.
• Experience as a Health Care Navigator or Certified Application Counselor.
