Physical Therapist (Outpatient)

Salary: $40.14–$58.21 per hour

What you will do

• Provide direct outpatient physical therapy and maintain responsibility for patient care from initial contact through discharge.
• Evaluate patients, select appropriate treatment modalities and protocols, conduct re-evaluations, and adjust treatment plans.
• Document evaluations, progress notes, daily treatment responses, and discharge summaries in the electronic medical record.
• Coordinate patient plans of care with physical therapist assistants and collaborate with physicians, nurses, case managers, and other healthcare professionals.
• Educate patients, address questions and concerns, and adapt treatment to individual needs and circumstances.
• Participate in committees, process improvement, departmental education, student clinical programs, and service development initiatives.

Requirements

• Bachelor’s degree in Physical Therapy.
• Active Physical Therapist license in the State of Illinois.
• Basic Life Support certification from the American Heart Association upon hire.
• Ability to meet department productivity, documentation, collaboration, and patient-care standards.
• Willingness to adjust the schedule or assignment to support patient care and departmental meetings.

Nice to have

• Master’s or Doctoral degree in Physical Therapy.
