Onsite Care Coordination Nurse (Healthcare)

Location: Hybrid role based at NKC Health Hospital, 2800 Clay Edwards Dr., North Kansas City, Missouri. Must live within a reasonable 30–60-minute commute of the facility. Limited remote work is available for coordination and documentation, with occasional travel to hirify.global offices for training or meetings.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support members and families with discharge planning, education, and post-discharge follow-up.
• Identify care gaps, safety concerns, and barriers to discharge, then develop strategies to address them.
• Coordinate care with attending providers, facility case managers, and other healthcare professionals.
• Educate members about diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete documentation and follow infection control, safety, and patient privacy standards.

Requirements

• Current, unrestricted Registered Nurse license in Missouri.
• Residence within a 30–60-minute driving distance of NKC Health Hospital.
• At least three years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in a hospital or health plan setting.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver’s license, dependable vehicle, vehicle liability insurance, private workspace, and internet service with at least 25 Mbps download and 10 Mbps upload speeds.

Nice to have

• Bachelor’s degree in Nursing.
• Experience in care management, health promotion, coaching, utilization management, or health plans.
• Bilingual English/Spanish skills.
• Care management or related specialty certification.
• Knowledge of HEDIS and Stars measures.
