Case Manager, RN (Denver, CO / Per-Diem / Field-Based)

Location: Denver, CO; field-based with frequent travel to patients' homes within an assigned geographic area

Salary: $45–$50 per hour

What you will do

• Conduct in-home clinical assessments and help patients and caregivers begin services.
• Develop, implement, and update individualized plans of care.
• Build relationships through scheduled home visits and regular clinical follow-up.
• Identify changes in condition, safety concerns, and barriers to successful care.
• Coordinate care with physicians, caregivers, clinical teams, and community resources.
• Educate patients and caregivers, review documentation, advocate for patients, and maintain accurate clinical records.

Requirements

• Graduate of an accredited nursing program.
• Active and unrestricted Registered Nurse license in Colorado required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently and collaborate with an interdisciplinary clinical team.
• Ability to travel by car within the assigned geographic area; valid driver's license and active auto insurance required.
• Comfort using clinical, case management, and productivity software.

Nice to have

• Experience in home care, community health, case management, or care coordination.
